Old un reduced posteromedial elbow dislocation: A rare case report

    This paper describes a very rere case of neglected posteromedial elbow dislocation along with its successful surgical management.

    The main claim of the paper is that postero-medial dislocation of elbow is rare. This is important because if indeed rare, this will give an insight into management of this condition.

    Yes, this case report is rare becausepostero-medial elbow dislocation is rare, while posterolateral dislocation is a common injury.


  • Are the claims properly placed in the context of the previous literature?

    Yes.


  • Do the results support the claims? If not, what other evidence is required?

    Yes.

    Not applicable.


  • Is the methodology valid? Does the paper offer enough details of its methodology that its experiments or its analyses could be reproduced?

    Yes.


  • Would any other experiments or additional information improve the paper? How much better would the paper be if this extra work was done, and how difficult would such work be to do, or to provide?

    The authors have not mentioned the status of the MCL and LCL intra-operatively and the status of the elbow joint on varus/valgus strain. This would have been done intra-operatively, but has perhaps not been mentione, but will add valuable information.

    Yes, because they have shown good functional outcome in a neglected case and especially without doing any soft tissue procedure such triceps lengthening etc.

    As such there are no claims but a good attempt to restore anatomy as well as function.


  • Are these claims novel? If not, please specify papers that weaken the claims to the originality of this one.

    These are novel claims as far as such case is concerned.


  • Are the claims properly placed in the context of the previous literature?

    Yes these claims are placed properly however some more cases of similar presentation could have been found in India. 


  • Do the results support the claims? If not, what other evidence is required?

    Yes to certain extent these claims are supported well but some more evidance could have been collected. 


  • If a protocol is provided, for example for a randomized controlled trial, are there any important deviations from it? If so, have the authors explained adequately why the deviations occurred?

    This being a case report these issues are not pertinent here. 


  • Is the methodology valid? Does the paper offer enough details of its methodology that its experiments or its analyses could be reproduced?

    Again being a case report the methodology does not matter. 


  • Would any other experiments or additional information improve the paper? How much better would the paper be if this extra work was done, and how difficult would such work be to do, or to provide?

    Intra operative photographs shpud have been included.


  • Is this paper outstanding in its discipline? (For example, would you like to see this work presented in a seminar at your hospital or university? Do you feel these results need to be incorporated in your next general lecture on the subject?) If yes, what makes it outstanding? If not, why not?

    The approach is standard whereas the case is rare posteromedial dislocation not so commonly getting presented after 2 months with such results. 


  • Other Comments:

    The case report is worth considering for publication as it gives good insight in management of such cases especially in Indian conditions. However it also highlightes poor primary management of such cases in this part of world if such case would have reported in tertiary care hospital immediatly after trauma then it would have not been worth reporting.
